Overnight Request Form

This form is the first step to reserve spots at MPM's overnights.

Please check this box ONLY if you are adding additional attendees to a previous request.

  • Use an addendum request when increasing your group numbers or filling in information missing from a previous submission.
  • Please be sure to enter the same group name as your original request.
  • This request is still subject to approval and may be denied based on availability.

Chaperones/Adults

We require a ratio of 1 adult for every 5 children

Please provide the following information for each Chaperone / Adult in your group:

  1. First and last name
  2. Phone number with area code

This information should be listed in the box below with one adult  per line and   a comma between each item.

This is an addendum request: Chaperone

  • Only type additional numbers above the original count to the "Chaperone Count" box if you are increasing the number of adults in your group.
  • If you are providing information about adults counted on a previous submission, please leave the count number as '0' and add new contact information in “Chaperone Info” field.
  • If you are adding a chaperone(s) to the original request, only type the number of additional chaperones in the number box, not the total amount.
Children

Children must be between 6 and 12 years old. We will not accept requests for children outside of this age range.

Please provide the following information for each child in your group:

  1. Child's first and last name
  2. Child's age on the date of the Overnight
  3. Child's emergency contact's name
  4. Emergency contact phone number with area code

This information should be listed in the box below with one child per line and   a comma between each item.

This is an addendum request: Child

  • Only add a number to the "Child Count" box if you are increasing the number of children in your group from your original request. If not, leave the number at 0.
  • If you are providing information about children counted on a previous submission, please leave the count number as “0” and add the new information in the “Child Info” field.
  • If you are adding a child(ren) to the request, only type the number of additional children in the number box, not the total amount.

In order to provide a fun and safe overnight experience, we require you to review the rules below and sign all appropriate waivers. Before visiting the Museum, please go over these general rules of conduct with your group:

  1. Payments: Full payment and accurate headcount are due four weeks prior to the Overnight. Groups reducing their size after their final payment are responsible for finding replacements for those participants. Please include confirmation number on your check.
  2. Cancellations: Reservations are non-refundable and non-exchangeable, unless the Museum cancels the Overnight due to enrollment requirements or weather. If the Museum needs to cancel, we can transfer your registration to another program date or offer a refund.
  3. The Museum has a predetermined schedule of activities for the evening and we require that you abide by Museum rules and respect Museum staff.
  4. Appropriate language and respect of others is expected of all adults and children attending the Overnight. Please remember that it is a privilege to spend the night at the Museum and that we must have your utmost respect of the Museum and the objects in our collection.
  5. There will be a mandatory safety orientation before bedtime. This will consist of basic directions—such as where the exits and bathrooms are—as well as instructions on what to do in case of an emergency or fire. We ask that your group give the speakers their undivided attention.
  6. Food is not allowed on the exhibit floors with the exception of water in a sealed container. You may store any food that you have brought with you in the school lunchroom on the Ground Floor.
  7. You may not bring any weapons, knives, matches, lighters, alcohol,  tobacco, or illegal substances into MPM. The use of any materials mentioned is prohibited within the Museum.
  8. You will be assigned a sleeping area. There is absolutely no bargaining or trading of sleeping areas with another individual or group. Areas were chosen for a number of reasons, and changing your sleeping assignment could compromise the safety of your group as well as the safety of those around you.
  9. It is strongly recommended that you do not bring valuables with you on your Overnight. MPM cannot assure the security of your personal items.
  10. Adults are responsible for ensuring the children in their group meet the age range policy of 6-12 years old. If children are not in the age range, you reservation request will be rejected.
  11. Please carry all emergency and allergy information for your group/family.
  12. Chaperones must be 21 years and older.

The Museum reserves the right to deny access or ask guests to leave if they are in violation of this agreement. If you fail to abide by the above guidelines, you may be asked to leave the Museum.

*I have read the Museum Overnight Agreement carefully and understand its contents. By submitting  this form, I agree not to hold the Milwaukee Public Museum liable for any personal injury or any and all damage, loss, or stolen personal property incurred while at the Museum.

We will review your request within the next 72 hours. You will receive an email indicating whether or not you have been approved. Once approved, please call 414-278-2728 to make payment and guarantee your space at the event.
